Annual Report

Accredited centres are required to submit an annual report to JACIE. The report is a brief summary of important changes (where they occur) in the accredited facilities and some information related to transplantation and quality management. A copy of the report form is available for download from the Applicant's section of the Document centre.

The report is presented every year on the anniversary of the accreditation being awarded (not the inspection date). The JACIE Office will contact the centre around this time to request the completed report.

Depending on the information reported, JACIE may request further information or a focussed on-site visit e.g. where a laboratory has moved to a new location.
In most cases, no further action is required.
