Dear applicant,
You have started the application process for JACIE Accreditation. The following sections of the web site will require you to have certain facts and information at hand in order to complete the initial application form. Since this form must be completed in one session i.e. it cannot be partially completed and saved, it is recommended that you compile the following information before you begin inputting data:
- CIC number of your centre (for EBMT members only)
- Full address, telephone and fax details for all facilities that are participating in the application.
- Name of the directors of the facilities that are participating and their email addresses Years of experience of the collection medical director
- Name of the designated person for quality management for each section (if applicable)
- Name of the data manager(s)
- Year transplant programme commenced
- Details of the qualifications of the programme director and medical directors of the collection and processing facilities if applicable
- Names of other attending physicians
- Number of inpatient beds
- Outpatients capacity
- Physician coverage
- Name of cell processing laboratory used by your centre
- Name of any other accrediting organisation applicable to your laboratories
- Transplant numbers performed at your institution covering the previous 24 months. These must be first-time patients.
- Number of collections for the 24 months prior to this application
- Names of Collection Facility staff
- Names of any other products collection by aphaeresis
- Cord Blood Collection Service Medical Director details if your centre requires accreditation for Cord Blood Collection
- List of components processing in the Cell Processing Laboratory
- Number of components processed per annum
- Names of other programmes served by the Processing facility Alternatively you can download a copy of the initial application form and print it for completion by hand. Then input the information into the online form.
